This form is to be used only by currently enrolled
VanderCook College of Music students to authorize credit
card payments for library services, including loan requests,
distance education services, and alumni/visitor borrower
fees, according to the following guidelines.
Charges for Interlibrary Loan Requests:
- Returnable Items (books, CDs, DVDs, etc.): $5.00 per item
- Photocopies (articles, book chapters, etc.): $.25 per page
- Rush requests: additional charge of $20 per item
- Late charges: overdue fines will be charged at the rates
set by the lending library

Charges for Distance Education Services:

Returnable Items owned by Ruppel Library (books, CDs, DVDs,
etc.): $5.00 per item
- Returnable Items not owned by Ruppel Library (books, CDs,
DVDs, etc.): $10.00 per item
- Photocopies (articles, book chapters, etc.): $.50 per page
- Late charges: overdue fines for items owned by Ruppel Library
will be charged our standard rate of $.25 per day. Items
borrowed from other libraries will be charged at the rates
set by the lending library
- Charges include shipping and processing to the student; Student
is responsible for shipping charges back to Ruppel Library

- Annual fee allows alumni/visitors to borrow materials owned
by Ruppel Library: $10.00 (does not include interlibrary
loan services)
