All-Catholic Nomination Form

We are testing a new form application.  Please ignore the dates at the bottom of the form.  The correct dates are as follows:
Registration Deadline:  Friday, December 22, 2017
Payment Due:  Friday, January 19, 2018

Student Nominations

Please list in priority order. Feel free to add comments if you wish.

*Students will be learning pieces that are Grade 2 1/2 - 4 in one day.  Please nominate students who will be able to handle the difficulty level of the pieces.

Submission Information

Completed nomination form must be received no later than Friday, December 23, 2016 A schedule of events will be sent for all students selected to the 2015 All-Catholic Honor Band. Once selection is complete a $40.00 fee (per student) will be due by Friday, January 20, 2017. Fees will not be refunded once student is selected, so please nominate wisely. Please pay all fees together with school purchase order or school check. A credit card payment is also accepted.  This fee includes a performance t-shirt and lunch. Music will be issued on the day of the festival.
